New Patient Registration

The practice will register new patients in the G11 and G12 postal districts.  You will be asked to complete a registration form and a health checklist. Both forms are available to download from the above "Registering Online" tab.

You will be offered an appointment with the doctor of your choice at which an assessment of general health, blood pressure, height, weight and relevant personal family history will be made.  You may be invited to follow-up assessments.

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the link below to open and edit the form. When you have completed all of the details, email your form to us at the email address below. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you are asked to fill out a medical questionnaire as it can take a considerable time for us to receive your medical records. This questionnaire is for both adults and children. You must also complete the pre-registration form above.

***Please ensure both the Pre-Registration Form and Online Medical Questionnaire are completed for both adults and children when registering online as you will not be registered until we receive both completed forms.***

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can register as a temporary patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the practice.

To register as a temporary patient simply contact the local practice you wish to use. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
